My unload macro is shift+Q, so I unload the gun if the mag is low and I'm not currently firing, press X to swap hands, click my pouch or belt on click to hand mode, click gun, let's rock.
Generally it depends on my gun and how forward thinking I am. If I'm feeling clever that round, I'd probably unload the half-used mag during lulls in combat, load a fresh mag, and then when I have a few half-empty mags just start moving bullets between them. If I have a shotgun, my rule is to load it with shells until it can't accept any more whenever I have a chance. For revolvers (including the Mateba, but this hasn't come up quite yet) I've set a macro to open the cylinder when I hit g, so whenever I run out of ammo I just hit z, g, x, click the quickloader, click the revolver, r, throw the quickloader, x, g, z.
Don't get me started on reloading a dropship though. That's a tedious process on its own. So is reloading the OB, so...

Unload weapon, use held item, swap hand macro. That I keep forgetting I've set up. I've used it roughly thrice in 4 years.
Unload macro, unwield, swap hand, choose mag of choice, slap it in.
In some cases click and drag mag to gun for tactical roads. (Usually when performing an action or when missing a hand, or just when too lazy to unwield)
The real pro's use tac reloads.
