Reason for Ban I can't remember, I think it had something to do with suciciding as Larva, but I can't recall the exact details, it was long before even my other ban on this server.

Your appeal, including evidence (screenshots, etc) I recently got my previous perma ban appealed after a little over a year of inactivity, I was hoping to get my job ban appealed in the same manner since its been so long and I'd like a second chance.
