Around 1 year ago when I was new to CM, Xenoban that was appealed.
Some time ago for EORG, instaban.
A week or two ago for killing a marine that stripped me and stole my SADAR.
Other:
1 and a half year ago on BeeStation. I later became the Head Administrator of the server, which shows improvement.

Your appeal, including evidence (screenshots, etc) I can no longer take screenshots as I'm banned from the Discord. I do remember the last message I sent though, which was along the lines of "this much drama over a pedo lmaooooooooo."
This was in no way harassment as I was simply stating that this whole situation shouldn't have happened to begin with. If the intent of the message was to harass the individual, it'd be in a setting where the individual was present. My messages never harassed any staff members or broke other rules (like +18 content as some people got banned over it.) I don't get how calling a pedophile a pedophile in an environment where they can't even see it is harassment.
