The short answer is never, HEFA should be thrown down ASRS and never touched
The long answer is that while HEFA has some niche uses it's so destructive and does so little damage to Xeno's outside of those niches that it does more harm to the marines then good.
RO's are encouraged to dispose of the M40 High-Explosive Fragmenting-Antipersonnel grenade at the earliest possible opportunity to minimize the impact on USCM operations against XX-121 Aliens

I always pack a HEFA on my chest. For protection? God no. If I know I'm about to be captured I'm priming that shit and running towards the xenos. When I do want to deny a push, I have a new strat I like to use. When the xenos are pushing and I don't want them to, I throw a training nade at them. It bleeps and already scares marines who aren't prepared for it, so I assume the same logic applies to aliens.
HEFA, much like Cluster OBs, were made by Xeno Rights Activists- thus they seem to be FF machines. Frankly, there are other grenades that just seem to be much better, especially when combined with the use of a launcher to actually put some distance between the boom and the user. People throwing grenades seems to be one of the main reasons why FF occurs. That and poorly timed pushes.
Fun fact about UGLs, you actually have shorter range when using a scope than without.
