An SO making a false insubordination is grounds for prevarication charges.

4. You are told by the CO that the Delta SL is to be arrested for insubordination for refusing to follow an order, and the squad leader tells you that they refused the order on grounds that it was an illegal order. What do you do and how do you procede?

Well, first I have to determine if the order was actually unlawful or not which isn't too complicated, if the order was lawful then the SL would be set to arrest for insub and we would go arrest if the location the SL is in is of convenience or if directly ordered by the CO. If the order was unlawful then I would just mentor the CO and maybe consider a fax if the circumstances were serious.
